A support plan is a tool that is used in order to establish the type of care and or support a client requires it also used as a guideline to be used by your personal budget assessor.

What We Provide
Stepping Out will help piece together a support plan step by step. We will help build a profile of the service user, and identify what kind of care & support is needed, and in which areas. The aim is to build an overview on the service user in order to propose the correct level of support.
